HOW IT WORKS
Three steps to a great Functional Assessment
1. CONTACT US
Get in touch by sending us an email. Let us know your situation, what you are hoping to achieve; and when you need it done by. The more info about what you need the better, it will help us to provide to provide you with a quote.
Our prices start at $1500 and range up to about $3000 depending on the level of detail you require.
You can either book an appointment, or send us an email to enquire further. Once have heard from you, we may gather a little more info from you over email or phone, and then we will send you a service agreement to confirm what has been discussed. You can take time to consider it, and if you would like to proceed, simply send it back signed.
2. TELL US MORE
Once you have decided to proceed, we invite you to send us other information that will help us to prepare for our appointment. Things that would help us with this include your last functional assessment, your NDIS plan goals, related medical reports or similar.
We will keep your details stored safely, and are committed to your confidentiality.
We encourage you to share your information with us so that we can read about your history before hand. Then during our time together, we can focus your valuable time and energy on the things that matter to you right now, rather than going over old ground.
3. REFINE
We will develop your report, and send you a final draft copy. We want you to read it through, let us know if your details are all correct.
We will have a final telephone call with you, to discuss our recommendations, answer questions, and make any final updates.
We will then email to you a final report copy; along with an invoice for payment.
